I want to know how I come across and where I could do better. Be direct. I'd rather have the real version than the polite one. For me, being kind matters much more than being nice. Specific examples help more than general impressions, but anything is welcome, even a single line. And if there's something I'm doing well that I might not realise, I'd like to hear that too.
